CougarConnect: Forms

On the Form Properties page, you can add and edit details about the form, including the name, visibility, when the form will be active, and if you would like only a certain audience to be able to respond to the form. Make sure to scroll to the top and select the blue button that says Save. 

1. Form Status - Make sure to toggle this to enabled when you’re ready for folks to start filling out your form.
    
2. Active from / to - Active From is the date and time your form will start receiving responses. Active Until is when it will stop receiving responses.
    
3. Include in Explore Forms List - If you want your form to be discovered and submitted by everyone, toggle this to enabled.
    
4. Require Review and Approval Process - If you want other people to review the submissions you receive, toggle this to enabled and you can assign reviewers. 
   
5. Allow Submissions from Public Users - You can toggle this to enabled to let folks respond to the form without logging in to CougarConnect. 
    
6. Allow Multiple Submissions - If you elect to have respondents log in, you can also allow them to submit responses multiple times by enabling this.
    
7. Submitter Identified Reviewers - If you want the folks submitting this form to submit a reviewer to double check their info first before you review it, toggle this to enabled.

Now you can add questions. There are 8 different question formats you can choose. You can also add multiple pages to the form by using the buttons at the top of the page. The form automatically saves your work.  When you have added all the questions needed, select ‘Back to Forms’ on the upper left of the screen.  To check your work and preview what your audience’s experience will be, you can submit a practice form. 

On this page is where you can begin to add questions. You can change details about the for m itself under Form Properties. You can change the details about each individual page on Page Properties. You can add additional pages as needed. You can also look at your Page List.

You can add 8 different types of questions. It's possible to embed videos into the question text as well. You will need to post the video on Youtube or Vimeo and then copy the link into the question text.

If there is an orange circle with an exclamation point in it, this means that you have recent submissions that need to be reviewed. 

When you select the 3 dots () next to a form in the Forms list, the following options are available:

1.  Edit the form questions - Please note that if you have already received submissions to this form, deleting a current question will delete the question and any answers from previous submissions. A tip to avoid this is to instead copy the form and change the questions in the new form. Make sure to share the link to the new form with your audience. 
    
2. Archive - Archiving old forms helps to keep your Forms section clean. Archived forms still hold onto submissions, but you have to activate the form in order to pull the data. 
    
3. Copy - This allows you to make an exact duplicate of this form. 
    
4. Share - This is where you will find the link to the form that you can then share with your audience via email, social media, on a flyer, etc.  *** Please do not copy the url from the web browser when you open your form because that will not take your audience to your form.***
    
5. Submissions - Allows you to review all of the form submissions. 
    
6. Export as Excel - Creates an excel sheet you can download to review all the submissions together at once. 
    
7. Export as PDF - Use this to download one large PDF document that includes every submission.

If you stop editing the form and navigate away, you can find it again by going to your user tool drawer (circle icon on upper right of the page that has your photo or initial) and then My Submissions.
