Equipment is for college use only.
I am responsible for maintaining equipment during the lending period.
I accept responsibility for the safety and security for the items(s) I am borrowing from Columbus State Community College.
I agree to return the item(s) on or before the agreed-upon date and time, and the items will be in the same condition as when I received them from the office.
