What is Forms?
Microsoft Forms is a simple, lightweight app that lets you easily create surveys, quizzes, and polls. In educational institutions, it can be used to create quizzes, collect feedback from teachers and parents, or plan class and staff activities.

Use Forms in a browser
Go to myapps.microsoft.com and locate, or search for Forms.

Click on Forms to open it in the browser for use.
How to use Forms
- Create a new form or create a new quiz. For this example, we will create a new form.
Note: If you are creating a form that will be used on behalf of a department or group, it may be best to use a group form. Reach out to IT Support for additional details.

- Add your questions and edit question settings.

- Adjust the form style and settings as desired. Preview and test the form to ensure it's ready.

- Choose Collect Responses. Adjust access for your respondents then share the form with them using the link, email, embed, or QR code options.

- View responses for the form or quiz. Export them to Excel if needed.
